What is an Exercise Physiologist?

Exercise physiologists use evidence-based practice to provide exercise intervention for chronic disease management, musculoskeletal injuries, and weight management. Exercise physiologists often work as part of a team of doctors, physiotherapists, and other allied health professionals to ensure the best treatment plan for the patients targeting their specific goals. They prescribe safe exercise and individualised programs to your specific needs. Areas covered by our exercise physiologist.

Our Exercise Physiologist

Susanne combines her clinical exercise prescription with a holistic mind-body approach. Susanne genuinely cares for her clients and the health of her local community. Her mission is

to support, educate and empower her clients with tools to achieve their unique health goals and optimise they're well being.   Susanne has specialised expertise in delivering safe and effective exercise interventions for clients with a broad range of health issues, injuries, and disabilities. She has a special interest in assisting clients with complex neuromuscular conditions, chronic pain, and mental illness.

 

Susanne has a Bachelor of Human Movement Studies (2007) and a Masters of Clinical Exercise Physiology and Rehabilitation (2018). Prior to founding Mindfulness In Movement, she has worked in Australia and internationally within multidisciplinary teams in hospital, corporate and private medical practice settings. 

 

Susanne has always been fascinated by the architecture of the human body but is equally intrigued by the power of my mind in healing. Her study, teaching, and most of all her personal practice of yoga and mindfulness has further deepened her understanding for the need to develop a powerful connection of the mind-body-breath to support one’s health, well being, and recovery. Collectively, this allows Susanne to offer her clients evidenced-based and holistic care.

What can I expect from my first session?

  • Your EP will undertake a comprehensive assessment in order to develop an exercise plan based on your unique requirements. This session will primarily consist of discussing your health, exercise, medical and injury history. By the end of this session, you and your EP will have a plan of action, including the start of an appropriate exercise program, and in some cases, a referral to another health or medical professional – just to make sure you’re working towards getting better, not worse! Assure you wear comfortable clothing as musculoskeletal assessments may be required such as checking, joint range of movement and muscle flexibility.

During your first follow up visit

  • Your EP will take you through your exercise program. They will guide you through exercises that will be of benefit to you, making sure you feel safe and confident to do them well on your own. You’ll receive a copy of your exercise program to take home and your EP will inform you of when a follow-up appointment would be most beneficial for you. This varies depending on the patients presenting injury or issues and individual goals. The follow-up appointments will be important to assure you get the most out of your program, correct technique for injury prevention and best results, the progression of exercises to make sure we use the FITT principle for best and fastest results.

Medicare – Chronic Disease Management

  • Chronic Disease Management Program is formerly known as (EPC)
    If you have a chronic medical condition you may get Medicare benefits for allied health services to help manage your condition.

  • Our Accredited Exercise Physiologists specialise in the delivery of exercise & lifestyle modification programs for people with chronic conditions. The Chronic Disease Management (CDM) program which was formerly known as (Enhanced Primary Care – EPC) is designed to provide specialised individual treatment services to patients who have a chronic medical condition and complex care needs.

  • Most Chronic health condition is subsidised by Medicare or your private health insurance

  • The Enhanced Primary Care (EPC) program was originally introduced to provide more preventative care for Australians and improve the coordination of care for people with chronic conditions and complex care needs. The program provides a framework for a multidisciplinary approach to health care.

  • All chronic conditions are subsidised by Medicare.

  • You may also claim on your private health insurance.

  • Our prices vary so it would be best to call ahead 

 

* There is a choice of either home visits or our personal training studio visits. Our personal training studio is the suggested venue for your consultations, but home visits can be arranged in some situations. Clients must be on an Enhanced Primary Care Plan to receive a Medicare Rebate.

 

How do I get started?
 

  • What you first need to do is speak to your local GP about whether you qualify for a CDM plan and then get a referral from your doctor.

  • Evolve Health and  Fitness Exercise Physiology Services is proud to collaborate with general practitioners in the delivery of high-quality allied health services. We offer a safe and innovative service that specialises in the delivery of exercise, lifestyle, and behaviour modification programs at our fully equipped studio as well as the patients' place of residence.

  • The following forms need to be taken to and completed by your doctor. You then bring these completed forms with you to the first exercise session, or your local GP may be able to fax the completed form to the studio.  A Medicare Rebate can only be received when the EP has the referral form.
